Here's how to get started: Click on each link below to open and print the forms. Fill out the information on each one and bring it to sign up night, or mail to: Susan Winn, c/o DYHA, PO Box 235, Douglas, WY 82633 along with your check for your child's age division.

We will allow families to post-date checks at registration in 3 installments if neccesary. You can check out equipment that night or call Kurt Gefroh at 358-2442 and he'll get you set up. The cost for renting DYHA equipment is $15, along with a post-dated deposit check for $100, to be torn up when gear is returned in the spring. Team jerseys can also be checked out with a $100 deposit, or purchased at Hometown Printed Apparel. One more thing you need to do is go to the USA Hockey website and register with them. (See link below) It costs $30, and is required for all team members (except MiniMites) to participate in Douglas Hockey. Print out proof of your registration and bring it to sign-up day. You can also view the Age Classification charts below to confirm which team your child will be on, according to their birth year.
